Understanding Environmental Drivers of Nurse Assaults


Violence in nursing workplaces has become an increasing problem in the American medical sector. Violence against nurses in their workplaces is not isolated but occurs in relation to environmental and mental needs.

Understanding how healthcare environments contribute to violence and their effect on both patient behavior and nurse mental wellness is critical to creating safer workplaces.

Types of Physical and Environmental Factors in Healthcare Facilities

Overcrowding in Hospitals

Conflicts can easily escalate in crowded hospitals and clinics. Delays and crowded waiting rooms can create frustration and stress among people. They can easily lead to violence, even in already stressful environments.

Health care facility design

Just as important as a pleasant atmosphere is a well-designed environment. A poorly lit, cluttered, or confusing environment could make individuals in the healthcare environment feel uncomfortable or imprisoned. On the other hand, a calm environment could make all individuals feel safe.

High-risk healthcare units

Some healthcare facilities are more challenging than others. For instance, emergency rooms, psychiatric facilities, and intensive care units are likely to have challenging situations; therefore, they can cause emotional distress and conflict. Nurses who work in these facilities are more likely to encounter violence.

Unsafe working conditions for nurses

Nurses often find themselves working long hours in stressful conditions, often alone or in remote areas of a medical building. Situational awareness that has been compromised by chronic stressful conditions usually makes them susceptible to violent attacks. Nurses may find themselves vulnerable due to fatigue and burnout.

Organizational Contributors to Nurse Assaults

Lack of adequate human resources

 Without enough nurses or basic resources on hand, the pressure mounts quickly. When the patient load is high, waiting and rushed care ensue. And for patients and families already experiencing stress, frustration quickly escalates into anger with nurses.

Poor security measures

A lack of security means that nurses feel vulnerable and unprotected. When security systems and security personnel are not in place, fear and anxiety become a normal part of working life rather than an exception.

Policies and training for violence prevention

Too often, nurses have to deal with violence without proper guidance. Violent situations get worse before anyone is able to intervene.

Violent Experiences within Nursing

In some workplaces, violence is simply accepted with a shrug and a dismissal that this is ‘just part of the job.’ Such an attitude undermines reporting and contributes steadily to burnout and a lack of job satisfaction. 

Ineffective leadership

A lack of communication from leadership after occurrences also leaves nurses feeling unheard and unsupported. This leads to a distrustful situation over time that contributes to stress levels, while concerns about safety are left unheard. 


Mental Wellness, Nurse Safety, and Violence Prevention 

Mental well-being and work safety are interconnected. It is essential to work on the environmental factors that lead to stress, and patient and nurse well-being initiatives to prevent nurse assault. It is for this reason that having safe environments at the top of the agenda is important not only for the protection of nurses but also for the establishment of healthcare facilities where mental health is addressed and violence is not normalized.

Overall, enhancing environmental safety and mental well-being can be pivotal in minimizing attacks against nurses and creating a safe healthcare system.
